What is known about tumour proliferation rates to choose between accelerated fractionation or hyperfractionation?

چکیده
Repopulation of surviving clonogenic tumour cells during fractionated radiotherapy is one of the crucial factors determining cure probability in radiotherapy. Clinical and experimental data suggest that repopulation rates vary considerably between different tumours but may be similar to the cell production rates in the untreated tumour. For those tumours which repopulate fast, such as squamous cell carcinomas, bladder cancer and colorectal carcinomas, accelerated fractionation may be indicated.

Tumour radiobiology beyond fractionation
Historically it has been shown repeatedly that single high doses of radiation do not allow a therapeutic differential between tumor and critical normal tissues but dose fractionation does. The purpose of conventional dose fractionation is to increase dose to the tumor while preserving normal tissue function. Tumors are generally irradiated with 2Gy dose per fraction delivered daily to a more or...
متن کاملDefinitive radiation therapy for moderately advanced laryngeal cancer: effects of accelerated hyperfractionation.
OBJECTIVE The purpose of this retrospective study was to analyze the results of accelerated hyperfractionation for patients with moderately advanced (T2 and T3) laryngeal cancer. METHODS Between 1998 and 2007, 9 supraglottic carcinomas (6 T2N0M0, 2 T2N2M0, 1 T3N0M0), 30 glottic carcinomas (25 T2N0M0, 5 T3N0M0), and 1 T2N0M0 subglottic carcinoma were treated with definitive radiotherapy using ...
متن کاملHyperfractionation as an altered fractionation regimen in primary radiotherapy for squamous cell carcinoma of the larynx.
The aim of the study was to investigate the efficacy of hyperfractionation as an altered fractionation treatment schedule in comparison with conventional fractionation in primary definitive radiotherapy for laryngeal squamous cell carcinoma. From March 1999 to December 2000, a group of 28 patients with previously untreated squamous cell carcinoma of the larynx were irradiated with conventional ...
متن کاملResults of radiation therapy for hypopharyngeal carcinoma: impact of accelerated hyperfractionation on prognosis.
We reviewed the results of treatment in 52 patients with hypopharyngeal carcinoma who underwent radical radiation therapy using a two-fractionation scheme--conventional fractionation (CF) and accelerated hyperfractionation (AHF)--in order to evaluate the impact of the fractionation scheme on prognosis. The 5-year survival rate for the 52 patients was 19.5%, and a significantly better rate was o...
متن کاملDOES HYPERFRACTIONATED RADIOTHERAPY CHANGE THE OUTCOME OF HEAD AND NECK CANCER? A TRIAL COMPARING CONVENTIONAL WITH HYPERFRACTIONATED RADIOTHERAPY
The optimal fractionation schedule for radiotherapy of head and neck cancer has been controversial. The objective of this randomized trial was to test the efficacy of hyperfractionation vs. standard fractionation. Patients with squamous cell carcinoma of head and neck organs were randomly assigned to receive radiotherapy delivered with A) standard fractionation at 2 Gy/fraction/day, 5 days...
